Purpose & Scope:
The Assistant Director of Case Management (ADCM) is responsible for assisting the Case Management Director (CMD) in managing the Case Management Program in order to ensure that identified and referred patients have a coordinated plan of care in accordance with Joint Commission and hospital guidelines. The ADCM will coordinate case management activities with ancillary departments and hospital liaisons and is accountable to the CMD. Skills should include: the ability to communicate with medical staff and hospital personnel at all organizational levels; excellent interpersonal skills and experience in or knowledge of issues related to patients of all ages.
Education:
Graduation from an accredited school of social work, nursing, or of health-related services is required.
Experience:
Two years of experience in case management in a hospital setting is preferred.
